Changing/Correcting Social Security Numbers in University Records
This process is for OSU Faculty, staff and students who need to update or correct their social security number in university records.
If you are currently affiliated with The Ohio State University, and need to correct an error with your social security number on record at the university, or update from a temporary OSU-assigned student identification/social sceurity number to a permanent social security number, please follow these instructions:
Students (and those staff planning to take classes) must change their information at the Registrar's office first. To do this, you may either go in person to the Registrar's office on the third floor of Lincoln Tower and fill out a Request for Change of Records [PDF], or you may fax the filled out form to the Registrar's office at (614) 292-8700. You will need a copy of your Social Security card and driver's license (or other government-issued photo id). If you fax your request, copies of these forms of identification must be faxed in addition to the Request for Change of Records [PDF]. The Registrar's office is unable to take these requests over the phone. Further information can be obtained by calling the Registrar's office at (614) 292-8500.
If you are currently employed by the University as a student employee, GRA/GTA, full or part time staff or faculty, you must register a change with the Tax Office. To do this, you may either go in person to the Gateway Building (1590 North High St., Suite 300)(292-2521), on the third floor, or you may fax a copy of your Social Security Card to the Tax Office. Their fax number is (614) 688-3640, and the fax should include a copy of your Social Security Card, your 8-digit Employee ID number (which can be found on your paycheck), your incorrect Social Security Number or temporary student identification number, your correct or newly issued Social Security Number, and your name. The Office of Information Technology will receive notification of the changes. If problems arise with the change, we will notify you by e-mail. If you wish to check on the status of the change, you may call (614) 688-HELP. Once we have made the changes in our system, it may take up to 48 hours to access secure websites, such as the Registrar's web pages, Fees and Deposits pages, and other University web pages requiring a login using the OSU Internet username and password.
You should not see a change in any services such as e-mail, downloading software from osusls.osu.edu, or logging into WebCT.
