For what purpose are the accounts needed? Please provide a description of the nature of the workshop/class and how OIT facilities will be used.< NOTE: Workshop accounts do not provide access to Carmen. If Carmen access is needed, please contact the OIT Technology Support Center at 8help@osu.edu to initiate a request for sponsored user accounts. This form is for Workshop accounts only, and can not be used for requesting sponsored user accounts.

What date(s) are the accounts needed?
We will need each user of a workshop account to sign and date an OIT "Policy on Abuse of Computers and Networks" form. This form will be attached to each account information handout that will be provided for the users of the workshop. The form can be signed during the first day of the workshop. The completed forms must be returned to: Sara Shriner OIT Customer Services 512 Baker Systems Engineering 1971 Neil Avenue We will delete the temporary workshop accounts 3 working days after the completion of the workshop (unless otherwise indicated).
